Job summary The Red House Surgery seeks a highly motivated, experienced Advanced Clinical Practitioner to join out dedicated, multi disciplinary clinical team providing a high standard of care for our practice population of 22,000 patients Our team is friendly, approachable and support each other well ensuring a pleasant working environment Main duties of the job The ANP role involves assisting the Duty Team with the daily triage, telephone consultations, face to face consultations and a daily home visit as required The successful applicant will demonstrate critical thinking skills to make safe clinical decisions, following practice policies and procedures. They will work collaboratively with practice colleagues to provide a high standard of care and meet the needs of patients. About us We are a well established, well organised, supportive practice in Cambridge, serving 7,800 patients. We have three partners and a team of salaried doctors, retainer doctors, Physician Associates, nurses, GPA's. physiotherapists, clinical pharmacists, social prescribers and social prescribers. We have a strong interest in teaching and training medical students and GP trainees. We play an active role in Cantab PCN. We have consistently high QoF achievement and are rated good by CQC. The three partners enjoy a close working relationship with the Practice Business Manager, Practice Manager and each other. There may be on occasion a requirement to carry out other tasks; this will be dependant upon factors such as workload and staffing levels. As a member of the duty team, providing efficiant and effective medical triage Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients in practice or in their homes for housebound patients Implement and evaluate individual specialised treatment plans for chronic disease patients Identify, manage and support patients at risk of developing long-terms conditions, preventing adverse effects on the patients health Provide advanced, specialist care to patients as required in accordance with clinical based evidence, NICE and the NSF Request pathology services as necessary Process and interpret pathology and other test results as required Maintain accurate clinical records in conjunction with extant legislation Ensure read codes are used effectively Maintain chronic disease registers Prioritise health issues and intervene appropriately Support the team in dealing with clinical emergencies Recognised, assess and refer patients presenting with mental health needs Extended and supplementary prescriber adhering to extant guidance Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required Contribute to practice targets (QoF, IIF etc..), complying with local and regional guidance Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc..)
