CLCH are seeking a motivated and enthusiastic Band 6 Dietitian to join our specialist Type 2 Diabetes community team on a permanent basis. This is an excellent opportunity to be part of an established multidisciplinary diabetes service, working alongside specialist dietitians, specialist nurses, doctors, healthcare assistants, and administrative staff in a supportive and collaborative environment. In this varied and rewarding position, you will provide specialist dietetic support to individuals living with Type 2 Diabetes across community settings, including GP practices, health centres, and sometimes client homes. You will be working across inner London boroughs, primarily based in Chelsea & Kensington, with the expectation to cover Hammersmith & Fulham and Westminster as needed. Your key responsibilities will include: One-on-one dietetic counselling for people with Type 2 Diabetes Delivering structured group education sessions, where appropriate Engaging in audits, service evaluation, and CPD activities to support professional growth To apply developed communication, negotiation and counselling skills to empower patients to achieve long term lifestyle and behavioural changes To participate in multi-disciplinary / multi-agency collaborative MDT meetings To be responsible for complying with legal and professional confidentiality guidelines at all times.
