Develop individualised holistic care plans which addresses nutritional status, weight, recommended dietary modifications, lifestyle factors, behaviour change, education, physical activity and where appropriate condition management. Support patient care through implementation of dietetic care pathways. Act as source of expertise in nutrition for medical staff, nursing staff and other healthcare professional.
