Job summary We are looking for an enthusiastic, motivated Nurse with an understanding of the benefits of early discharge and rehabilitation within a familiar home environment. Someone who can work within a Multi-disciplinary Team, using their initiative to make decisions around complex issues. The successful candidate will promote and implement the Discharge to Assess principles and pathways. They will identify, assess and co-ordinate services to support the safe discharge and transfer of patients with complex needs, liaising with the Multi-disciplinary Team and partner agencies across acute and community settings. You will be either be a qualified Registered Nurse, Occupational Therapist or Physiotherapist. Closing date: 02 June 2025 Interview date: 16 June 2025 Main duties of the job The band 6 Discharge and Transfer lead will be responsible for up to x 4 wards within a bundle and will be the lead for complex discharge in those areas. You will be also mentoring band 4 and band 3 discharge support officers. Main duties are to assess and liaise with the multidisciplinary teams to ensure the patient is on the correct discharge pathway, the role entails completion of transfer of care documents and also attended meetings when required in relation to discharge. To ensure patient discharge care needs are assessed, planned implemented and evaluated using research evidence as appropriate. To provide leadership to facilitate excellent nursing or therapy care in order to promote rehabilitation, patient independence and therefore earlier discharge from hospital. To ensure all nursing or therapy procedures are carried out in accordance with UHDB hospital policies taking appropriate action in emergency situations. In conjunction with the multidisciplinary team, be responsible for setting, monitoring and maintaining excellent standards of care particularly for patients with multiple needs who require complex packages. Maintaining personal contact with patients, relatives and carers being sensitive to their needs for courtesy, dignity and privacy and ensuring a friendly environment at all times.
