Job summary Children and Family Health Devon (CFHD) is delighted to advertise for a Band 5 Mental Health Practitioner to join our Assertive Outreach Team, part of the Urgent Care Pathway. The Assertive Outreach Team works with children and young people who are at risk of hospital admission or stepping up into more intensive services, offering intensive community support to reduce crisis and promote emotional recovery. This is an opportunity to be part of a small, dynamic and trauma-informed team offering high-quality, evidence-based care that keeps young people safely connected to their families and communities. The service operates between 9am and 8pm Monday to Friday, and 9am to 5pm on weekends and bank holidays, offering flexibility and genuine teamwork to meet the needs of the children we support.
