Job summary We are looking for two highly skilled and motivated individuals to lead our Rother & Wealden Neighbourhood Community Mental Health Teams as a result of our Trust wide Community Transformation Programme.The transformation of community mental health services will bring local services together to work more collaboratively, including mental health, social care, primary care and voluntary services. Main duties of the job The Neighbourhood Mental Health Teams are currently being established, which as a minimum, will bring together the existing primary care and VCSE service for mental health (Emotional Wellbeing Service), specialist community based mental health services (Assessment and Treatment Service and Specialist Older Adults Mental Health Services), and Access Facilitators, to create a new multidisciplinary service.
