Job summary Dementia and Older People's Mental Health Liaison service is a specialist Team of Doctors, Nurses, Occupational Therapist and Support Workers working within the hospitals in Cornwall. This is an exciting opportunity to work alongside general medicine using a biopsychosocial framework. This role will be part of a supportive, friendly and dynamic team. The current Team include a variety of staff with different backgrounds who are collaborative, inclusive, supportive, nurturing and progressive. There will be training opportunities and the option to develop the service through quality improvement. There are opportunities to liaise with the wider South West Liaison services. Proposed interview date: to be confirmed Main duties of the job The Dementia and Older People's Mental Health Liaison provide mental health assessments and care planning services to patients: Patients of any age with a diagnosis of Dementia. Patients over the age of 75 with a functional mental health diagnosis who require additional specialist support and assessment, to include: Depression Schizophrenia Anxiety Bipolar Disorder We use a variety of assessment tools such as the ACE-111, 4AT, HADS, I Cared, I Shared and Short IQ Code, alongside professional judgement and discussion with the wider MDT. We help to formulate Care Plans, assess risks and support the acute hospitals in managing this. We link with families and carers, medical staff including Geriatricians, voluntary services, and specialist services such as palliative care, Parkinson's Team, speech and language therapy, dieticians, frailty team, alcohol liaison and ward based staff such as Ward Nurses, Occupational Therapists, Physiotherapists and support workers. We work towards the safe discharge of patients through adhering to legal frameworks with positive risk taking, working closely with the wider Community Mental Health Services, Adult Social Care and a number of voluntary agencies. We also offer a 7 day follow-up review on discharge from hospital. We have access to and manage specialist Mental Health Discharge to Assessment beds which allow further assessment outside of the hospital environment.
