Job summary The Transfer of Care Hub at Colchester Hospital is looking to recruit a Quality and Flow Coordinator to join our developing team. The aim of the discharge team is to maximise personal independence and enable patients to return home as quickly as possible in a supported manner. The Quality and Flow Lead will be responsible for providing continuous data reporting required to ensure the service is deliveringhigh quality assessments in accordance with the service specification, including KPIs throughplanned and ad hoc reporting on the services clinical and quality performance in an accurate, clear and timely manner. This is a fixed term/secondment position until the 31st of December 2025 Internal applicants currently employed by ESNEFT will be offered on a secondment basis only. Main duties of the job As the successful candidate, you will: Contribute to the development of a data quality culture within the service by highlighting data issues and working to develop solutions and ensuring routine audit and validation of all data. Be an ambassador for driving change within the service through participation and continual improvement activities Identify, analyse and interpret trends or patterns in data sets for continued evolution of the Transfer of Care Hub and its services Collaborate with stakeholders to develop, review and communicate quality assurance regarding policies and processes, ensuring alignment with best practices whilst supporting with the development and production of policies and procedures relevant to the Transfer of Care Hub. Support with producing benchmarking data against the NHS England Discharge to Assess Model Support improvement planning processes working closely with Senior Management to identify action plans and ensure that ESNEFT governance is adhered to within the Transfer of Care Hub About us We are ESNEFT and we provide hospital and community health services to almost one million people across east Suffolk and north Essex.
