Job summary Closing Date: x04.06.25 (this may change dependent on response) Shortlisting to take place after closing date: commencing 05.06.25 Interview expected to take place in the week following shortlisting: commencing 16.06.25 The Vascular Care Coordinator will provide a comprehensive administrative service to support patient pathways and cover multidisciplinary teams (MDTs) to ensure the effective management of accurate and timely data to enable clinical decision-making at MDT meetings when required. The post-holder will coordinate, proactively manage and track patients from and to Bradford NHS Foundation Trust from Airedale NHS Trust and Calderdale and Huddersfield NHS Trust, ensuring IPTs are completed and sent to the respective Hospital. They will escalate problems to the management team in a timely manner so that remedial action can be taken where necessary to ensure the 18 week RTT targets are met. Main duties of the job The role will involve scheduling of patients, maintaining an accurate and up to date view of the vascular virtual ward, working collaboratively with radiology, theatre management team and secretarial staff to maximise efficiency in the patient's vascular pathway.
