Job summaryCommunity Mental Health TransformationNew Older Adults TeamCommunity Mental Health Social WorkerBased at one of four locations Stratford, Coventry, Nuneaton, Rugby (County wide post)Do you want to be part of a brand-new service as part of Coventry and Warwickshire's Transformation of Community Mental Health Services?We are developing a new team to work with Older Adults experiencing functional difficulties..The service is being developed in conjunction with health, social care, statutory and voluntary sector partners to have an integrated service with partnership working. There will be close links with the Dementia Service to work towards providing an excellent service for all older adults.Our new team is taking shape, and we want this to be an integrated multi-agency and multi-disciplinary service. We currently have OT's, Nursing, Psychiatry, Psychology, community Social Workers, Voluntary sector, support workers within the team and we work alongside our VCSE colleagues and experts by experience.Main duties of the jobAs a social worker in the team, we will ensure that you have clinical and professional support to support the maintenance of your professional registration. This support will be alongside the supervision and management that will come from the Trust giving the best of both worlds.The successful applicant will work alongside the leads and colleagues to continue to develop the service and begin to pick up a specific relevant clinical caseload, and provide evidence based interventions as well as liaising with other agencies.We are looking for enthusiastic, dynamic and experienced individuals who can work confidently on their own as well as being a team player. Good communication and problem-solving skills are essential, as well as knowledge of mental illness in the Older adult population.
