Job summaryThe Tissue Viability Team in Southampton is offering a fantastic opportunity for a highly motivated and enthusiastic Band 5 nurse to join our service. The role involves working in a complex wound/leg ulcer clinic.The successful candidate must be able to demonstrate an interest in tissue viability, with current experience in pressure ulcer, leg ulcer and chronic wound management. You will ideally have experience of working in a community setting. You will take pride in your work and be highly motivated and dynamic, have excellent communication skills and a flexible approach and be able to embrace change.The ideal candidate will be dedicated, highly motivated and committed to providing high standards of care to people within clinics, their own homes and the local community. You will have excellent communication skills as you will be undertaking complex nursing assessments, you will also be required to build relationships within the specialist teams to help deliver a high quality service. Experience in community nursing is desirable for this post but not essential as full training will be provided and the post holder will be supported to develop in the role. The Southampton Tissue Viability Team are based at the Adelaide Health Centre and will see patients in the community, Community Hospitals, GP Practices, Nursing Homes and in clinic.Our role is to support health care professionals in clinical practice by providing teaching, information, and advice. The service is for clients who have tissue viability needs and are registered with a GP in the Southampton area.We provide:*Complex wound assessment and advice*Leg ulcer service, including lower limb assessment*Telephone advice*Training and education*Supervision and competency review*Link nurse meeting and trainingThe purpose of this position is to meet the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Care is based on comprehensive assessment, ensures continuity and is patient/client centred, culturally sensitive, evidence-based and which promotes independence.About usWe have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services.
