Job summaryNHS@Home Coordinator - Band 4Location: Beacon House, Hopton Park, Devizes21 Hours over 3 days a week - PermanentShift times: Early shift 8am to 15:30 and Late shift 12:30 to 20:00. One in four weekends (enhancements for weekend work and two days off in the week)We are looking for an experienced administrator to work closely with our Wiltshire NHS@Home Virtual Wards service joining a multi-disciplinary team.The team provide care to the acutely unwell who would otherwise be admitted to hospital, but for whom short-term treatment at home would be a better option. Treatment will include face-to-face assessment, care, and monitoring, with daily consultant-led virtual multidisciplinary team (MDT) reviews for each patient.Main duties of the jobThe NHS@Home Coordinator role will include liaison with our Community Teams, Urgent crisis response and our Acute partners providing coordinated administration for a virtual ward model. The work will include arranging and coordinating admission to the virtual ward, coordination of patient daily reviews and writing up the outcome of appointments and booking future sessions. To undertake other routine administrative tasks as required (e.g., note taking, formal minute taking, meeting room booking, document scanning, filing, document formatting, assisting the with the rota, ordering, photocopying, etc).
