Job summaryCroydon & SuttonSWL ICB - All Age Continuing Care teams have recently been organised into 3 teams each covering 2 local authorities and are responsible for the delivery of ICB statutory responsibilities of the National Framework for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) [ revised July 2022]. This post is an exciting opportunity for the successful candidate to build on existing management skills and knowledge of CHC to deliver a first class CHC service and be part of the change in service to reflect the All Aged Continuing Care Service required by NHSE.This post reports to the Deputy Director for CHC at Place and will be expected to deputise for them as when necessary including representing the SWLICB locally engaging with parties statutory and non-statutory and at NHSE.The successful candidate requires a registered professional qualification in health or social care with excellent communication, previous experience of working within and managing a CHC clinical team, driven and motivated for the challenge of moving the team forward to deliver an All Aged Continuing care service. The SWLICB requires the service model to deliver a high-quality service that is robust and framework compliant.Main duties of the jobThis position is a senior leadership role responsible for overseeing and managing the operational delivery of the assessment, decision making and commissioning of care packages for those assessed as eligible to receive Funded NHS Continuing Healthcare and FNC.
