Job summaryThe role of this post is to support the North Manchester Hospital at Home Team. The clinical level is expected to be at IMT3 or equivalent, although this role is not recognised as an official training post by the JRCPTB or Royal College of Physicians.The candidate must have a desire to work in the field of general and acute internal medicine, The candidate should have a good experience, competencies and background of this area and be prepared to work independently with the support of consultants.The Hospital at Home service has two parts: one is remote monitoring and telemedicine and the other provides clinical care & therapy in the patient's home. In both streams patients are monitored regularly and their care continues to be managed by an Acute Medical Consultant. If required patients may also be brought back to the Same Day Emergency Care Department (SDEC) for secondary care investigations and clinical reviews. Should it be identified a patient is deteriorating these processes provide safe patient care, allowing quick intervention and if required re admission to hospital. The Hospital at Home team includes Acute Medical Consultants, GPs, Advanced Clinical Practitioners, Specialist Nurses, Occupational Therapists, Physiotherapists and Pharmacists.North Manchester General Hospital (NMGH) is expanding the Hospital at Home service to support safe patient care, placing the right people in the right location.Main duties of the jobProvide supervision, assistance and instruction to the junior medical staff e.g. Foundation trainees.Maintain the highest clinical standards at all times and in accordance with accepted good medical practice and the various clinical policies of the Service.Transmit relevant clinical information to other medical, technical and nursing staff both within and external to the department, regarding patients referred to other places for further treatment of follow-up.Supervise nursing staff in matters relating to the treatment of patients.Teaching and supervision of medical students, nurses and junior hospital at home team members.Occasional duties may arise at the acute trust, e.g. major incidents, ensuring adequate ward staffing.The post holder accepts that they will also perform duties in occasional emergencies and unforeseen circumstances at the request of the appropriate Consultants.The post holder is expected to provide cover for the occasional absences of colleagues as far as practicable.The post will consist of a basic working week of 40 hours plus on callAbout usMFT is England's largest NHS Trust with a turnover of £2.6bn & is on a different scale than most other NHS Trusts.
