Job summaryAn exciting opportunity has arisen for an enthusiastic, experienced Integrated Sexual Health Practitioner to join our sexual health outreach team, providing specialist contraception and sexual health advice and support to vulnerable young people age 11-25 years. This person needs to be dynamic and work independently, to ensure the service is provided in a timely and sensitive manner.The successful candidate will work closely with the Youth Engagement and Participation Service (YEPS) and the Resilient Families Service (RFS). As an outreach post the successful candidate will need to travel across the RCT area to provide care in a variety of settings.The Sexual Health Role will sit within the Youth Engagement & Participation Service (YEPS) to work in partnership to deliver an equitable outreach provision across RCT for 26hrs per week between Monday to Friday including a minimum of two evenings per week as required by YEPS. Occasional events may require working weekends to support the YEPs team.The support will be delivered in a range of settings including schools, colleges, youth and community venues, as well as mobile and street-based locations, virtually and face to face support, providing a coordinated approach in delivering of the outreach provision to increase the young person's accessibility to Sexual Health services.The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.Main duties of the jobThe post holder will deliver targeted specialist contraception and sexual health advice and support to vulnerable young people age 11-25 years in order to:Improve the sexual health and wellbeing of young people in RCT;Reduce the rate of STIs in RCT;Continue to reduce the rate of unintended teenage pregnancies in RCT;Promote awareness of child sexual exploitation;Reduce risk-taking behaviours and encourage positive lifestyle changes.They will provide a nurse-led flexible, targeted, sessional outreach service within the local community, specifically targeting those young people who are unlikely to access mainstream provision.The post holder will provide specialist support that will compliment universal provision, and not duplicate the work of other professionals working in the field, e.g., school nurses, youth workers etc.They will contribute towards the outcomes of the new Families First Performance Framework, and the key priorities of the Resilient Families Programme (RFP).The post holder will be accountable for own professional actions, within the boundaries of the NMC Code and organisational policies and procedures.
