Job summary Are you looking for a new challenge. Ward 14 is a 29 bedded ward with patient who have complex surgical or social needs. We are looking for an enthusiastic team member to help with the flow of the patient's journey. You will be working closely with the nursing team and bed management team to aid discharges for patients on ward 14. Main duties of the job *Work closely with nurse in charge of the ward and the MDT for discharge of patients and aid flow. *Completion of reports/documents with up-to-date patient information *Update trust computer systems with relevant patient information *Communicate with other agencies eg: social services or district nurses *Communicate with patient and/or NOK. *Arrange transport for discharges of patients.
