A Vacancy at Rotherham Doncaster and South Humber NHS Foundation Trust. As part of ongoing development and expansion we are developing our Hospital Liaison Team based at Doncaster Royal infirmary. The team is an all age service and we have older adult and substance misuse staff embedded within the core team. We welcome applications from individuals with a minimum of 2 experience of delivering high quality assessments alongside evidence based interventions to adults receiving care within our local acute hospital. This post is an exciting opportunity to make a difference work in a developing service at the front end of physical and mental health care in the acute hospital setting. This role will enable you to make a real difference to the lives of local people, by offering assessment, intervention, liaison in relation to co-existing mental health and psychological issues. The service will work closely with physical health, community mental health and IAPT services as well as our local social care and non-statutory colleagues. The role calls for a highly skilled individual with excellent assessment, communication and brief intervention skills. The successful candidate must enjoy working as part of a wider team and fully understand the importance of liaison with other services. An awareness of how physical and mental illness can impact on outcomes and quality of life is a must in this position as well as good negotiation and problem solving skills. To understand the requirements of the Mental Health Act 1983, Mental Capacity Act 2005, Care Act 2014, Care Programme Approach, Deprivation of Liberty safeguards as it applies to a psychiatric liaison service. To promote and assist in the maintenance of the highest attainable standard of nursing care in a general hospital setting, drawing on examples of current best practice. To provide specialist support and advice to referring agencies and case management, preventing unnecessary delays. To maintain timely clinical records and share content of clinical records/information appropriately with other professional and agencies, working across interfaces, to improve communication and timely patient care. To advise patients, carers and significant others of the available resources. Referring assessed need to those agencies best able to meet these in the community and facilitate use of these resources.
