Job summaryAn exciting opportunity has arisen to work as part of the multidisciplinary team to develop and deliver a high quality clinical pharmacy service to patients on the Older Persons Assessment and Liaison OPAL (Frailty) Unit in line with local and national objectives.We are looking to appoint an experienced and motivated clinical pharmacist. The ideal candidate will be an innovative and enthusiastic team player who communicates effectively at all levels. The candidate should have a post graduate clinical qualification and/or significant relevant clinical experience to demonstrate a similar level of knowledge in Care of the Older People. They should be self motivating with a drive to develop medicines optimisation and clinical servicesMain duties of the jobAs part of the Pharmacy team on the OPAL (Frailty) Unit you will:work closely with the Lead Frailty Pharmacist to provide a 7 day pharmacy service as per agreed shift pattern (12 hour shifts)Support transition of care of patients from the Frailty Assessment and Short Stay Unit to other wards.Provide excellent patient care through direct patient facing activities.Provide leadership on matters pertaining to medicines optimisation, to advise on medicines use (risk, clinical governance, research and development, formulary, expenditure) and to develop/audit relevant protocols and guidelines.Be responsible, on a day to day basis, for medicines reconciliation and medicines optimisation for patients on the Frailty Assessment and Short Stay Unit who will be admitted onto a medical or surgical ward at PAH.Undertake clinical supervision and training of junior staff.Provide support and in-reach to other ward areas where necessaryAbout usOur OrganisationThe Princess Alexandra Hospital NHS Trust (PAHT) provides a full range of general acute, outpatient and diagnostic services at The Princess Alexandra Hospital in Harlow, the Herts and Essex Hospital in Bishop's Stortford, and St Margaret's Hospital in Epping.We employ 3,500 staff and serve a local population of around 350,000 people living in west Essex and east Hertfordshire, centred on the M11 corridor and the towns of Harlow, Bishop's Stortford and Epping.
