Job summaryAssistant Practitioner Home First Discharge Coordinator - Band 4North Wiltshire Locality - Chippenham Hospital37.5 Hours per week - PermanentIf you love working in a Community setting we have a great new opportunity for a Band 4 Assistant Practitioner to join Chippenham Community Team. This post could also be a developmental opportunity for you to undertake an Apprenticeship to complete a Health and Social Care Foundation Degree.The role is based in Chippenham Community Hospital but there will be a requirement to travel to the North Locality Community Teams regularly. This includes Marlborough, Malmesbury and Calne. The role is primarily 08.00-16.00 Monday to Friday although you may, on occasions, be required to work other shifts including weekends.As an Assistant Practitioner you will be focused on coordinating our Home First service. supporting, coordinating, and reviewing patients being discharged from hospital requiring rehabilitation.Main duties of the jobThis role provides support and co-ordinates the Home First referrals to the North Locality teams to ensure a coordinated approach to facilitate timely hospital discharges. This is across 4 community teams determining capacity and sourcing therapy input. The role includes working with locality MDT to promote group screening of referrals and deciding on discharge categories.We also work closely with the wider Multi-Disciplinary team, Nurses, Acute Hospitals, Primary and Social Care and Locality Hubs.Applicants will need excellent IT skills including excel and good communication and organisational skills.
