Job summaryWe are looking for a professional, enthusiastic, highly motivated Clinical Pharmacist who can work independently to join our team and contribute to developing our patient services.The successful candidate will utilise their patient facing clinical practice and independent prescribing qualification to work alongside our established clinical team to support our medicines management team, manage patients with chronic disease and develop their skills in seeing and treating patients as part of our multidisciplinary team.Duties will include the ability to evaluate treatment/interventions and provide care for patients with undifferentiated and undiagnosed problems using clinical assessment skills in the Primary Care setting.Main duties of the jobThis role will work as part of the team to provide specialist advice to patients, and undertake full patient centered medication reviews to proactively manage people with complex polypharmacy, particularly the elderly and those with multiple conditions.Work in general practice will also involve medicines reconciliation, management of prescribing queries and acute medication issues following hospital discharges, and the development and implementation of safe systems for medication ordering and prescribing in the context of improving safety and reducing waste.Participate in dealing with acute prescription requests and review of medications at transfer of care.Advice on safety of medicines,quality improvements, participate in clinical audits and sharing learning events.About usLeiston surgery is a friendly, innovative and well-organised surgery that is looking to recruit a Clinical Pharmacist to enhance their clinical team.The doctors and staff aspire to provide traditional family medicine and the highest quality of care. The four GP partners all have special interests and actively encourage the clinical team to develop their own skills.The Clinical Team currently consists of two Physician Associates, two Practice Nurses, four Health Care Assistants, Nurse Prescriber, Social Prescribing Team and Physiotherapists.Job descriptionJob responsibilitiesThe post holder will provide primary support to practice staff with regards to prescription and medication queries and help support the repeat prescription system. They will deal with acute prescription requests and medicines reconciliation on transfer of care for patients. They will support the development and delivery of systems for safer prescribing, providing expertise in clinical medicines.You will have excellent IT skills and communication skills.The post holder will be an independent prescriber and be able to consult with patients as well as be proficient in repeat prescribing systems. The postholder will also need to maintain a good knowledge of QOF, enhanced services, QIP and other developments in relation to General Medical Services ContractUndertake clinical medication reviews with patients and make recommendations on prescribing and monitoringPromote self-management and develop patient support systemsManage therapeutic drug monitoring system and recall of patients taking high risk drugs i.e. anticoagulants, anticonvulsants and DMARDs etcWhere appropriate and required, provide patient facing clinics and / or phone consultations for those with questions and concerns about their medicines in the practiceMonitor practice prescribing against national guidelines and formulary recommendations.
