Job summaryCommunity Dementia team 22.5 hours per week.We have an exciting opportunity for a Qualified Nurse to join our well-established Community Dementia Team which is a countywide service. The team provides low-level support to individuals with a dementia diagnosis and dementia assessments.The service is open Monday to Friday 9-5 excluding bank holidays. Flexible working patterns will be considered.The team is based in the grounds of the Belmont Abbey, which is peaceful rural location, 2 miles from the main city of Hereford.
