Job summaryAn exciting opportunity has arisen for highly motivated individual to join our Community Team within South East Shropshire in the provision of Planned and Admission Avoidance care to patients in the community.We are looking for a caring, motivated and friendly individual to join our busy, professional and dynamic team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care.The post holder will assist and support other members of the team in giving care / undertaking phlebotomy to patients within a designated area, under the supervision of a qualified nurse.The opportunity to develop additional skills within the HCSW job description and support in career progression through accessing formal training and clinical competence.Main duties of the jobPromotes a 'can do' attitude and determination to develop and sustain a dynamic and responsive clinical and administrative support service within a Community Hospital / Community Team setting which delivers best practice.To deal appropriately with interruptions, unpredictable events and behaviours.To organise and deliver skilled clinical support work / phlebotomy without direct supervision.Gain valid informed consent and work within legal frameworks with patients.To liaise with nursing and therapists with regard to patient care, highlighting any variations to expected outcomes from agreed treatment programmes.To provide basic verbal and written clinical reports to the supervising Nurse and / or therapist and to document patient interventions/findings in patient records in line with Trust Record Keeping Policy.About usShropshire Community Health NHS Trust provides community-based health services for adults and children in Shropshire, Telford and Wrekin, and some services in surrounding areas too.
