Job summarySouthern Health NHS Foundation Trust is seeking to recruit a full-time Band 5 Learning Disability Staff Nurse to work in the North and West Hampshire Community Learning Disability Service.This is an exciting time to join a busy and well-established multi-disciplinary health team supporting the health care needs of people with a learning disability throughout the North and West Hampshire Locality. You will play a vital role as part of the community nurse team in a forward thinking and innovative team. We offer regular team days and nurse meetings, supervision and opportunities for professional development including leading and participating in local and divisional quality improvement initiatives.We are looking for a newly qualified or more experienced nurse who can manage their own caseload, complete assessments within the areas of clinical practice and develop robust person centred care plans. You will lead nursing care for service users referred to the team using the CPA process when indicated. You will be supported to supervise, mentor and teach unregistered staff and students. This post will give you the opportunity to contribute towards leadership within the nursing team and wider multi-disciplinary team. You will need to demonstrate effective leadership and inter-personal skills, good communication and presentation skills, be organised and self motivated.Main duties of the job*Whilst completing a period of preceptorship, work under the direction and clinical supervision of a nominated preceptor.*To assess, plan, implement and evaluate the nursing care needs of a specific group of patients.The assessments and interventions undertaken will be embedded within the framework of:MDT principles of workingThe holistic model of care (Four Tier model)The six areas of clinical practice*Once preceptorship is complete, to supervise, mentor and teach qualified and unqualified staff including students*To co-ordinate care for a specified group of patients*Be a member of an integrated inter-agency, multi-disciplinary learning disability team.*Contribute to effective multidisciplinary working within the Learning Disability team and build effective liaison with community, primary, other secondary and tertiary services and agencies to meet individual need and service development.*Manage a caseload of patients under indirect supervision and be accountable for assessing, interpreting, planning and implementing nursing treatment and care to patients within professional guidelines.*Ensure that systems are used to incorporate any adopted models of quality assurance and governance into everyday practice.*Act on behalf of, and report to the Charge Nurse as required.About usSouthern Health is one of the largest NHS Foundation Trusts in the UK, specialising in mental health and learning disabilities, as well as offering physical health community-based services.With a workforce of more than 7000 and a footprint that spans more than 200 sites across Hampshire, we're wholeheartedly committed to providing the best possible healthcare, reaching a range of diverse communities, whilst placing patients and staff at the forefront of all our endeavours.We are currently working closely with other NHS Trusts to integrate all our collective community, mental health, and learning disability services.
