Then this role may be for you.Main duties of the jobWe are looking for an experienced mental health professional to join our ServiceAccess Team as a Community Mental Health Co-ordinator, within North TynesideTalking Therapies. The role supports patients from the point they access the service,through the assessment process and into treatment. You will also liaise bothinternally within the service and externally with other agencies to ensure that clientsreceive the best possible care. We have one of the most extensive patient experience programmes of any trust in England.Job descriptionJob responsibilitiesInvolvement in all aspects of service access including referral triage, enhancedassessments, risk assessments, clinical pathway decision making and liaising withexternal services to support client needs.
