Job summaryThe service mission is to provide a high quality service for people with severe and enduring mental health needs by working in active partnership with service users, carers and with local voluntary and community groups, in addition to statutory services within the Primary Care Network (PCN) Structure in Newham. The 9 Newham PCNs are grouped into 6 CIMH Teams, managed by a North and South service manager.Main duties of the jobTo link with a number of GP practices and with the community recovery team catchment area teams and Consultant Psychiatrists linked to the PCNs, providing Primary Care colleagues with advice and support on cases they are managing, including carrying out assessments at the GP's request.To gather information from GP surgeries, CRT's and prepare in organising provision of assessment, treatment of service users with mental health needs in the primary care settings and to supervise primary care support workers including Peer Support Workers, Community Connectors etc. To be an active member of the multi-disciplinary team and be responsible for a caseload of people with mental health problems who are being managed by Primary Care, including the administration of Depot medication;About usOur mission is to make a positive difference to people's lives by improving the quality of life for all we serve.
