Job summaryCome and join our fantastic District Nursing team in Frome. East Mendip, Somerset.We have an exciting opportunity for a newly qualified or experienced Nurse to work as part of our friendly and dynamic multi disciplinary care team. You must be an enthusiastic individual with excellent communication skills and motivated in your approach to care delivery to ensure we maintain a high quality service for our patients.You will be supported to manage a caseload and working as part of a larger multi disciplinary multi agency team, liaising closely with our Primary Care Networks and Neighbourhood Teams as part of the out of hospital model of care.The community nursing service is commissioned to provide services between 8am and 10pm, seven days a week, including evenings, weekends and bank holidays. Applicants will be required to work a shift pattern within these hours as part of the role although flexibility can be discussed. A driving licence and access to a car is essential.Main duties of the jobCommunity Nurses work as part of a team providing high quality and compassionate care to individuals. The Community Nurse is an autonomous practitioner who is responsible for the holistic assessment of both complex and simple care needs, the development, implementation and evaluation of care and delegation of care where appropriate to other members of the team. They will support the multidisciplinary team in the delivery of high quality care to enable individuals to achieve their person centred goals.The Community Nurse will take responsibility of the patient's care pathway and non --clinical duties in the absence of the Team Leader. They will have the understanding to address complex problems in their day-to-day work, reporting concerns to a Team Leader.The Community Nurse will be able to work effectively as part of a team and will always act within the limits of their competence, knowing who to ask for help and support as required.
