Job summaryFelixstowe Community Healthcare Team has an opening for a permanent fulltime (37.5 hours per week) B5 Community Nurse.The Felixstowe Community Healthcare service is part of a wider Integrated Neighbourhood Team (INT) consisting of district & community nurses, generic workers, community matron, occupational therapists, physiotherapists and Social Care Workers led by a B7 registered health team lead and overall managed by an INT manager.The team is busy, dynamic and proud of their holistic care approach. The team continues to change to support developments in NHS which includes close working relationships with the local statutory, non-statutory and voluntary services including GP, acute hospital colleagues, GP Federation,Social Prescribers, SuffolkMind, NSFT and many local community voluntary groups.You will be actively supported by regular supervision and individual annual appraisal. You will be expected to work autonomously and on behalf of the patients served by the local area. You will have access to skills development training relevant to this post.The role involves providing nursing interventions to the housebound residents of Felixstowe and surrounding area, who are 18+ and registered with a Felixstowe GP. Our main aim is to support, maintain and encourage people to make healthy lifestyle choices in the ageing well process.Main duties of the jobTriage, assess, plan, implement and evaluate all aspects of nursing patient care, and develop care plans which may need to be delivered from a range of options.Ensure informed consent is obtained and documented prior to initiating interventions.Following a holistic assessment, devise an individualised, evidence-based plan of care for each patient, modifying it as required.Assess patients' holistic needs, communicating complex and sensitive information to patients and carers.Use tactful and persuasive skills to gain the patient's co-operation in their care management plan.Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated.Maintain skills with regards to latest technologyParticipate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework.Ensure own workload is planned and prioritised according to patient needs.Ensure patient held records are completed for each visit, and that SystmOne inputting is completed daily.Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills.About usWe are ESNEFT and we provide hospital and community health services to almost one million people across east Suffolk and north Essex.
