Job summaryThe Droitwich, Ombersley and Rurals Neighbourhood Team is an innovative and collaborative team. We are looking for an Occupational Therapist who is keen to deliver high quality evidence based care in the community setting. The post is available both full time and permanent. You will be part of a team who provide essential knowledge and skills to support patient discharges and prevent unnecessary hospital admissions.As a part of the team, you will be instrumental in the journey of patients on the planned and reactive care pathways, such as the intensive support pathway. This will involve assessing patients who are being discharged from hospital from the acute and community hospital settings as well as patients who require support to prevent admission, identifying the appropriate intervention such as short-term care support while the patient rehabilitates as well as any immediate equipment needs, to prevent admission. The role is fast paced and responsive to urgent referrals and works closely with other nursing and therapy roles. Making a patients home safer and guiding the promotion of independence is where the Occupational Therapist really adds value to the patient experience and that of the colleagues within the neighbourhood team.Main duties of the jobWe have a team of creative health care professionals focused on compassionate and goals focussed care. We work closely with our GPs, Social Services, Specialist Nursing teams and other neighbouring clinical areas, developing relationships with other primary care services, utilising skills of our MDT colleagues to manage complex patients ensuring services are responsive and patient centred.A day shift is 7.5hrs between the hours of 07:30 18.30, Monday to Sunday including bank holidays). You will be expected to work varying shifts to cover the needs of the service, which rostered several months in advance. Working weekends will attract an enhancement payment. We can also offer Flexible Working Patterns to offer a greater work-life balance.Your typical day will include working closely with nursing colleagues to manage the daily Huddle, Intensive Support Pathway, weekly MDT meetings, in addition to coordinating your caseload, triaging referrals, allocating workload and prioritising urgent response assessments alongside colleagues.You will have opportunities for further training and development within the role. You will be completing assessments for patients for low level equipment, adaptations and manual handling needs.Initiate and co-ordinate individual rehabilitation programmes and support and educate assistants to deliver these effectivelyUndertaking occupational therapy assessments for a designated caseload, addressing occupational performance and skill deficits, enabling the client in areas of self-maintenance, productivity and leisure.Assessing clients: in relation to their equipment and specialist equipment needs / for minor adaptations eg rails, half steps.Working with clients to identify Occupational Therapy goals as part of the overall care plan.Monitoring and evaluating treatment in order to measure progress and ensure effectiveness of the intervention.Act as case co-ordinator for patients on the intensive support team in order to co-ordinate their ongoing needs by liaising with the patient, GP, MDT, relatives and other agencies.
