We support staff in clinical practice in many ways.We will provide training, shadowing opportunities and support you to work autonomously in the community and use your own initiative. The successful candidate will have been able to coordinate complex care packages, have good administration and IT skills, as well as a good working knowledge of CPA processes, report writing, organisational skills, will have care manager role, safeguarding knowledge and look to lead the team in this area with support and the ability to identify issues and demonstrate ownership of change processes.
