Job summaryBe at the heart of our community!A rare opportunity has arisen for a Community Specialist Practitioner within our friendly Community Integrated Care Team in Upper Lea Valley!Main duties of the jobThe Specialist Practitioner will work within the Integrated team of Community Nurses, Therapists, CPNs and Social Workers. You will have responsibility for ensuring all health and social care needs are met to improve health outcomes, functional status and quality of life. You will be patient focused and have the ability to undertake clinical assessments to arrive at a differential diagnosis, start, plan and evaluate care. You will also have organisational and time management skills and be able to demonstrate leadership skills across the inter-professional team.
