Job summaryThis is your opportunity to join a forward-looking community team, if you have previous experience in this area of nursing or if you are new to this specialty.The Community Nursing Team works as part of a wider multi-disciplinary team, caring for the residents in the Knutsford area. We provide high quality, sometimes complex care to all housebound patients in our area, including those in residential homes. We enable patients to manage their long-term conditions whilst remaining at home & preventing unnecessary admissions to hospital. Our work enables us to integrate with other services in the community including virtual wards & crisis response.The impact of our multi-disciplinary team is most evident when we provide palliative & end of life care. We are also on the brink of launching a new service to provide all care for these patients in their last year of life. This wraparound care will be provided in partnership with East Cheshire Hospice & is unique in its development.Hours per week: 37.5 (part time will also be considered)Main duties of the jobThe successful candidate will have excellent, up to date clinical skills & be enthusiastic, motivated and be able to work flexibly across our patch. It is expected that candidates will be able to contribute to the on-going advancement of the team & will be rewarded with a friendly, supportive environment in which to develop.The nursing team is part of a wider care community in which we support Knutsford residents to live well & we work with other agencies to support residents with activities, befriending, advice & exercise opportunities.Applications are welcomed from Band 4 Registered Nursing Associates & Band 5 Registered General Nurses. Experience is not necessary & enquiries are welcome from those interested.
