Support begins at the point of recruitment and throughout your role to gain competence and confidence.We are part of a dynamic, integrated health and Social multidisciplinary team delivering high quality care, advice, and support and monitoring to patients, their families and staff within the community settings. You will be involved with the development of INT working and leading on new initiatives to prevent admissions and facilitate discharge. The service runs 7 days a week, bank holidays and weekends.
