Job summaryThe community team comprises a cohort of experienced practitioners from medical, nursing, social work, occupational therapy and psychological therapy backgrounds.There is access to 3 wards, one in each of the boroughs: Holbrook at the Woodlands Unit in Queen Mary's Hospital, Sidcup which is a dementia ward with 19 beds, Shepherdleas Ward for functional illnesses at Oxleas House, Queen Elizabeth Hospital, Woolwich and Scadbury Ward at Green Parks House, Princess Royal University Hospital, Orpington. Patients are admitted to the wards from the Community teams via the gatekeeping Intensive Hub.Main duties of the jobMental health provision in the community to residents above the age of 65 is the responsibility of the Older People's Community Mental Health Team. The team is fully integrated with RBG social services and provides health and social care for older adults on CPA. The post holder will be based at the Memorial Hospital working alongside the other two consultants who provide sessional input to the team.The service also has 15 functional continuing care beds at the Memorial in Oaktree Lodge and 18 dementia continuing care beds in a purpose built home in Kidbrooke opened in 2004. . Community teams include CPN's (9) CPN assistants (2) Psychologists, OTs and Social Workers (5). We also have a trust wide Intensive Hub as well as excellent and well developed Memory Clinics. Memory Clinics work within NICE guidelines and have a specialist dementia nurse as well as consultant level medical input. Greenwich Old Age services also have joint specialist clinic for patients with combined Parkinson's disease and mental illness.
