Job summaryAre you looking for an exciting opportunity to work with Diabetes Services?Rotherham Diabetes Specialist Nurse Team are searching for a dynamic, motivated Diabetes Specialist Nurse who will be responsible for working in partnership with young people, providing advice and support to help them to manage their condition. The Diabetes Transition Specialist Nurse role is an exciting, autonomous post and the post holder will have a sound knowledge of diabetes care with excellent communication and negotiation skills. The ability to challenge practice is essential. The role of Diabetes Transition Nurse includes working collaboratively with the, Adult, Paediatric and Obstetric Diabetes teams to provide a seamless service for young people. The post holder will be able to work autonomously, be flexible and be able to use digital and social media as part of their role. They should also have experience with diabetes technology such as Insulin Pumps and Continuous Glucose Sensors.The post holder should be a non- medical prescriber/willing to work towards the prescribing qualification. The post holder will also be required to deliver educational packages to health care professionals and patients around effective diabetes management.The successful applicant will not only have excellent clinical skills, but will be able to demonstrate the ability to work and make decisions autonomously in a clinical setting but also to function within a multi-disciplinary teams.Main duties of the jobDevelop skills and support young people with the use of Diabetes technology. To scope, design and review the clinical transition pathway for young people embarking on transition to adult services, improving care and outcomes for all young people moving into adult services.To work alongside the Clinical Leads in Paediatric, adult diabetes and obstetric services. To support the Young Peoples diabetes service and attend the MDT young peoples clinic. To support the Diabetes obstetric service and attend the MDT Diabetes in pregnancy clinic. To demonstrate improved clinical outcomes for young people with diabetes by developing best practice to ensure that young peoples specific needs are met, to reduce variation in service delivery and reduce risk for young people transferring between paediatric and adult services. To liaise with paediatric and adult services to develop practice to ensure that young people who have diabetes have a positive experience throughout the service.In collaboration with the paediatric and adult diabetes MDTs, actively advocate for young people in their preparation for transition and during the transition period to adult services.Contribute to clinical governance within the Trust in transitional and obstetrics care using national and local guidance and benchmarking of services.
