Job summaryA Highly profile, accessible and professional working autonomously, with clinical responsibilities for a defined group of patients. Responsible for direct liaising with GP's, external agencies, community colleagues, Ambulance Services, ICB and clinical speciality teams with UHCW to ensure appropriate and timely referrals. Able to make informed decisions within their own area and scope of professional practice to ensure agreed standards, objectives and targets are met. Possess some knowledge of admission avoidance/ambulatory pathways/services both with UHCW and the community.To work closely with staff in the Medical Assessment Unit, the Emergency Department, GP's, Urgent Treatment Centre, and the Site team whilst managing efficient, effectiveness and safe delivery of patient care within Same Day Emergency Care (SDEC)/ Support the flow of suitable patients from ED to SDEC incline with SDEC pathways.Please be advised that this job advert will close as soon as sufficient applications have been received.Main duties of the job1.Co-ordinate timely access for all emergency Gp referred patients to the hospital, contributing to the clinical prioritisation of emergency patients, deciding on appropriate admission arrangements to match the activity within the Emergency Department and overall demand of the trust.2. A knowledgeable practitioner, operating agreed procedures for the safe and effective use of the Trusts Emergency Department capacity. MAU/SAU capacity and the Trust capacity in Emergency/Fast track clinics, identifying issues which prevent the safe and effective management of emergency Gp referrals, planning, negotiating, and agreeing access.3. Effectively communicate information regarding clinical decisions, policy and care pathways to patients/carers and the multi-professional team including situations where difficulties incomprehension occurs.4. To offer telephone advice to patients and their carer's, GP's, and allied health professionals.5. Will contribute towards corporate objectives, acting with local, trust and statutory guidelines and polices at all times.Navigate IT systems to secure accurate and timely patient, workforce, and resource data available. Willing to undertake ALL IT courses.Early identify Frail patients referred by Gps and community teams and alert Fraiity SDEC (Same Day Emergency Care) as soon as possible to help smooth the patient's length of stay.Working with WMAS SDEC to continue to promote the service to help the patient be in the right place at the right time.Regularly manage expected and unexpected clinical events requiring high but unexpected levels of physical effort according to patient dependency/clinical need.
