Job summaryHealth Care Support Worker - Band 3Chippenham Community Team30 and 29.5 Hours per week available - PermanentIf you love working in a Community setting, we have a great opportunity for a Health Care Support Worker to join Chippenham Community Team. This role will be for 30 hours per week worked over 4 or 5 days.The team is based in Chippenham Community Hospital and provides patient care between 07.00 22.00 hours with shift patterns including some weekends and late shifts which can be flexible. The team supports patients who are registered with GP practices in the Chippenham locality, and we work closely with the wider Multi-Disciplinary team, carers, and voluntary services.The role is very rewarding and will include supporting patients and providing effective nursing-based interventions in patients homes and other community settings. Our caseload includes patients with palliative care needs, tissue viability needs and a range of acute and long-term conditions. This role is also linked to our Urgent Care and Virtual Ward service so you may be required to support our Advanced Clinical Practitioners.Main duties of the jobYou will need excellent interpersonal and communication skills and will be expected to have a Care Certificate and Level 3 Diploma in Healthcare Support or be willing to work towards this. Wiltshire Health and Care is also proactive in developing community specific training and can offer a supportive and friendly environment in which to learn new skills, a range of training and development opportunities, including an induction program.
