Job summaryWe have a newly created post in our Community Hospitals service for a Lead Dementia Nurse to join us and provide Kent wide leadership across the whole health economy.The Kent Community Hospitals provides re-enablement and recovery for patients following a period of illness or an acute hospital stay.This role will see you provide clinical expertise and strategic direction and guidance for achieving excellence in practice whilst supporting the Head of Service in the delivery of the Dementia/Delirium and cognitive impairment service across Kent.Main duties of the jobYou will will be responsible for providing professional leadership to senior nurses within area of expertise and professionally manage and support the dementia nurses, across Kent in the delivery of a high quality and evidence-based service.You will act as the clinical expert for advice and support to both clinical staff and the wider multidisciplinary team, whilst offering clinical supervision to the cardiac nursing serviceYou will work to stablish an effective workplace culture across the dementia service that sustains person centred, safe and effective care through self-awareness, leadership, active learning,development, improvement and innovation.About usRated 'outstanding' by the CQC, we've got a passion for delivering high standards of patient care and excellent services to improve the health of our communities.
