Job summaryThe post holder will be a registered midwife working as a member of the Lotus Team for Vulnerable Patients. This team promotes continuity of care for patients who are socially vulnerable, including those with mental illness, substance misuse, teenage pregnancies, refugees and asylum seekers and supporting the prevention of pre-term births. There will be opportunities for training and development within these specialist areas for the successful applicant.The post holder must be able to work autonomously, be responsible for managing a caseload and ensure they are referred for appropriate interventions and properly safeguarded.The post holder will be both hospital and community based and work alongside the Specialist Midwife for Perinatal mental health/Lotus team leader, Substance and Alcohol Misuse Lead Midwife (SAMS) and Obstetric Lead for Perinatal Mental Health to meet the individualised needs of these vulnerable patients. They will also be required to work closely with external agencies such as the Perinatal Mental Health team, Change Grow Live and Family Nurse Partnership.
