Providing medications to patients for discharge to ensure that they can take their medication easily.To support the management of ward medicines in line with the Trust medicines optimisation policy.We are the Medicines Management team based at Basingstoke and North Hampshire Hospital and the Royal Hampshire County Hospital in Winchester. We are part of the wider pharmacy Clinical team that work across the hospital covering admissions, medical, surgery, respiratory, stroke & elderly care wards.
