Job summaryThis exciting post will be working in the Memory Assessment Service (MAS) providing assessment, potential diagnosis and signposting to other services and support networks for individuals aged 65 and over referred to the service with cognitive impairment.We are looking to recruit a Band 6 Memory Assessment Nurse.The posts will work in the areas with the highest waiting times and will work with colleagues to reduce the waiting times in that area. There will be a requirement to travel between sites throughout Nottinghamshire.Main duties of the jobTo work as part of the multi disciplinary team to screen for appropriate referralsTo complete an assessment of clinical presentation and symptomsTo share information amongst the multi disciplinary team and create appropriate and person centered care plansTo refer for appropriate diagnostic procedures eg scans, neuropsychology, occupational therapy.To make independent diagnostic decisions based on consultation, history, presentation, pathology and/ or imaging results and to seek supervision from service medical support in complex diagnostic situations.To complete accurate recordsAbout usNottinghamshire Healthcare employs over 10,000 colleagues who help #MakeADifference every day.
