Job summaryWe are looking for an advanced Nurse Practitioner to join our multi-disciplinary team at Wincanton Medical Centre, which is part of Symphony Healthcare Services.The hours available are 37.5 hours per week on a Permanent contract. Working pattern to be discussed at interview. The Salary range is £40,221.81 - £56,216.67 per annum pro rata with an NHS or Nest Pension offered.Main duties of the jobThe Nurse Practitioner will work within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures. In order to work at this level, NMC requirements for advanced practice must be met.About usWincanton - Our Practice & TownThe practice supports over 9000 patients and the large multidisciplinary team includes a highly skilled acute team of Nurse Practitioners and Paramedic Practitioners who manage all on-the-day demand, acute home visiting and the minor injury service for the locality. We work closely with our practice teams to support, develop and transform patient care and working practices with innovation and technology at the heart of what we do.Job descriptionJob responsibilitiesClinical PracticeAssess, examine, diagnose, plan, implement and evaluate treatment/ interventions and care for patients presenting with an undifferentiated diagnosisProactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management planPrescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practiceWork with patients in order to support compliance with and adherence to prescribed treatmentsProvide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactionsPerson SpecificationQualificationsEssentialRegulated Health Care professional with current registration (NMC/HCPC).At least 5 years post qualification and registration: - including at least 6 months working within primary care and/ or urgent care setting.Minor illness module level 6 or above.Physical Assessment of adults course level 7 or above.Palliative care/ End of life care course or module.Verification of Expected Death course.Consultation Skills- (history taking and physical examination.)Registered as a non-medical prescriber with the relevant regulator (V300 non- medical prescribing qualification).Supervision/leadership/mentoring training or qualification.DesirablePhysical assessment of children or equivalent module.Chronic disease management.Recognised Teaching and Assessing course.Mental health and/or learning disabilities qualification.Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.UK RegistrationApplicants must have current UK professional registration.
