Referrals come from multiple internal and external sources. CRR are working in conjunction with our ambulance service to increase referrals into CRR for 2-hour Urgent Crisis Response as set out by NHS England and NHS Improvement.Our team won the 2021 Nursing Times Award for care of the older person.The team consists of:Nurse Consultant for the Older Persons ClusterAdvanced Clinical PractitionersRegistered NursesPhysiotherapistsOccupational TherapistAssociate PractitionersNursing AssociatesAdminThird sector supportMain duties of the jobThe successful applicant will be responsible for delivering high quality individualised patient centered care using a holistic approach.
