Job summaryAn exciting opportunity has arisen for a community rehabilitation band 6 Occupational Therapist to join a supportive and enthusiastic community therapy service, working across community delivering rehabilitation in patients own homes following discharge from an acute bed, or supporting Long term condition managementThe team comprises of qualified physiotherapists, and assistant practitioners, and assistant therapy staff, who work closely with the wider MDT and INT teams in community.We have appointed a new Trainee Advanced Clinical Practitioner in community and there is an opportunity to develop an interest in frailty and long term conditions.You will be working with patients with a large variety of conditions and disabilities, in their own homes and local community. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff and provide support and expertise to the other members of the team.We offer a fabulous training programme within a supportive teamThe service operates across 6 daysPart time working consideredMain duties of the jobThe post holder is an autonomous practitioner, responsible for provision ofspecialised assessment and treatment of patientsreferred to the service. . You will see patients in own homes , working with an MDT in other organisations.
