Our team work on the principle of home is always the first option and we pride ourselves of working towards this ethos however not all patients are suitable for home discharge and this is where we at the Integrated Discharge Team come into our own.If you are an enthusiastic, experienced registered professional with specialist knowledge and experience and are looking to further develop skills in discharge assessment & planning then we have the role for you in our Integrated Discharge Service.Main duties of the jobThe operational manager will have responsibility for the effective coordination of patient discharges throughout the Trust. You will be part of the leadership team within the integrated discharge service. They will play a key role ensuring that wards comply and embed the standards and processes in place, utilising 'estimated date of discharge' and 'criteria to reside' to support safe and timely discharges across the 7-day week. They will ensure 'Home First' principles are adopted, ensuring patients are assessed and supported to maximise their independence.You will be highly organised and reliable to ensure effective functioning of this service. The service is an essential component to support management and detailed understanding of the Trust & wider health economy's capacity. Be responsible for completing department sickness reviews, manage annual leave, staff appraisals, performance reviews and will support the operational lead for discharge to deliver within agreed budgets.As part of the role, the successful candidate will be required to work on either Trust site.
