Job summaryWe are a new team looking for an enthusiastic team player to join us as a pathway coordinator.The post holder will provide coordinated care for patients with Severe Multiple Disadvantage (SMD) . The post holder will be responsible for providing a high quality communication and liaison service across a wide range of services determining and signposting service users and staff to appropriate services that support integrated, multidisciplinary working.This requires the receiving and processing of referrals from a range of agencies and effective communication across professional organisational boundaries, ensuring equality of access across full range of services in a timely manner in lines with agreed protocols.This post is a critical part of the newly formed SMD team.Please note that this post does not meet the pay level required for a Skilled worker visa.Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa.
