Job summaryThis is an exciting opportunity for a Patient Flow Assistant to join a small and experienced team working across Division 1 (Surgical Division). You will be able to work on your own initiative and seek help as required. You will join a team of two experienced surgical flow co-ordinators and training/support will be provided.Main duties of the jobProviding an efficient administrative service to discharge and transfer co-ordination of Patients1. To lead on co-ordinating the discharge or transfer arrangements of medically and therapeutically stable patients identified as medically fit for discharge within a designated area. The post-holder will participate in the assessment of after-care needs of clients and relatives/carers, contacting where necessary other members of the multidisciplinary team. The post-holder will independently co-ordinate the safe discharge and transfer of patients back into the community or into 'step down' facilities.2.
