If so, then come and join our Home First programme.Within Wiltshire Health Cares successful Home First programme we have a vacancy within the Warminster and Westbury Community team based at Warminster Community Hospital.The Home First Service provides therapy, care and support in peoples own homes for the first few weeks after discharge from hospital, and rehabilitation to promote independence in activities of daily living.You will be required to hold a level 3 Health and Social care apprenticeship (or equivalent) or be willing to work towards.We want to meet applicants keen to develop into this rehabilitation role or who have existing transferrable skills. Our support workers are also required to undertake the care certificate if they are do not already hold this qualification.Applicants need to have enthusiasm and commitment to working within a community-based team and to have recent experience of working in or in partnership with community services within the NHS or Social Care.Main duties of the jobIt is envisaged that the majority of the work will be between 8am and 9pm, split into shifts of 08.00 14.00 and 15.00 21.00 for part time workers, 7 days a week. You will also be given mobile devices to support you to be able to provide a responsive and person-centred service 7 days a week. This post is for 30 hours per week over 5 days.We want enthusiastic, motivated and hardworking people who can work flexibly and efficiently.
