This will include working alongside the PCN Pharmacy team and GP supervisor to support our Care Home residents.Responsible for managing and running long-term condition clinics, with the possibility of running these clinics in our Extended Access service.As required, be able to prescribe for the caseload of patients within your proven scope of practice.
