Job summaryWe have an exciting opportunity for an experienced Enhanced Nurse to join our Primary Care Network (PCN). The post holder will be the focal point for and work closely with our Primary Care Network member practices and clinicians to ensure that the Shropshire Rural Alliance PCN delivers the contractual elements of the Network Direct Enhanced Service (DES) contract.Main duties of the jobThe successful candidate will be working at an enhanced level within primary care and will be able to deliver quality individualised care to our patients. This will include working alongside the PCN Pharmacy team and GP supervisor to support our Care Home residents.Responsible for managing and running long-term condition clinics, with the possibility of running these clinics in our Extended Access service.As required, be able to prescribe for the caseload of patients within your proven scope of practice.
