Job summaryWe are seeking a Social Prescriber to join our practice of 9400+ patients.15 hours per week and must be available for afternoon shifts. This role requires 1 day in practice and 1 day working from home.Bridge House Medical Practice is open between the hours of 08:00 to 18:30 Monday to Friday. The successful candidate will need to be hard-working, flexible and enthusiastic. We are looking for a capable individual who is able to multi-task and work well under pressure.Main duties of the jobAs the Social Prescriber, you will provide follow on support to service users from the assessment and care plan, or from interactions with other professionals such as the GP. You will provide holistic assessments and co-design social prescription to improve health and well-being outcomes of individuals. In particular, you will:1. Focus on helping patients to make full use of local community and voluntary sector services available locally, as well as those provided by health and social care providers.2. Manage a caseload of patients through holistic assessment to onward referral by providing ongoing support for a short-term timeframe to promote engagement with identified services and achievement of targets.3. Update fellow teammates and members to increase their awareness and use of local services, develop new opportunities, and problem solve to improve access.About usWe are a training & sponsor practice, located on the Haringey Green lanes and you will need to live within easy commuting distance.This is a great opportunity for an enthusiastic, well organised and adaptable person to work within a friendly and supportive primary care team.You must have your covid vaccine to join as a member of the practice.Job descriptionJob responsibilitiesPrincipal TasksService deliveryTo take referrals from GPs and other members of the team for individual patients.To discuss with the person their needs, based on GP guidance, and to direct them to appropriate social prescription servicers.To manage a caseload of clients through holistic assessment to onward referral, working with clients in the practice as identified by the GP. This will include provision of ongoing support for an allocated but short-term timeframe to promote engagement with identified services and achievement of goals.To develop knowledge of local services, using existing databases and developing links with service providers, keeping up-to-date with service changes and developments.To inform and develop GPs and other healthcare professionals on how to identify patients suitable for social prescribing service referral. The aim is to support the development of care planning and case management to support patients in their management of care and avoid unnecessary use of primary care and hospital services.To actively participate in practice level multi-disciplinary team meetings.To work in partnership with all voluntary and community organisations to build a comprehensive database of local resources to design and support the Social Prescription activities.To train and develop GPs and primary care health teams knowledge on how to identify patients suitable for social prescribing service referral on a quarterly basis.To set up and maintain comprehensive data and evaluation systems, including individual health outcome tool (such as Recovery Star).Leadership and management of peopleTo support and supervise volunteers (as assigned) to ensure performance targets are met.To lead by example, upholding Bridge House Medical Practice codes of conduct, policies and working practices.To lead by example by modelling healthy living practices whilst in work.Wider Community involvementTo increase the participation and involvement of residents in the activities of local services and other practice activities.To contribute to work with local communities to build and sustain community capacity and seeking local solutions to community identified issues and priorities;
