Job summaryAn exciting opportunity has arisen for a Band 5 Staff Nurse to join our Transfer of Care Hub. We are looking for an enthusiastic and experienced Nurse who has excellent communication and assessment skills, is passionate about patient flow and able to work at a fast pace in both a changing and challenging environment.The successful candidate will be responsible for completing assessments for patients to facilitate discharge from hospital through a Discharge to Assess model. We are looking for individuals who wish to achieve best patient outcomes on discharge from hospital embracing the Home First approach.Hours per week: 25.5 hours.Maternity leave cover, fixed term contract until the end of June 25.Secondments will also be be accepted, please ensure permission is obtained from your line manager before applying.Main duties of the jobThe post holder will facilitate initial proactive assessments for inpatients in order to facilitate a safe, effective discharge from hospital and prevent avoidable readmission to hospital.Key aims areDelivering a consistent, coordinated approach to assist patients to identify potential needs at the point of discharge to avoid unnecessary delays in transfer of careWorking collaboratively with external and internal partners for the benefit of the patients to ensure smooth transition from hospital and reduce risk of readmissionTransparently providing information and expert advice to patients, carers and other professionalsReporting all delayed transfers of care, for criteria to reside reporting .About usWhether you are just starting out in your career or are looking to use your skills and experience in a new role, East Cheshire NHS Trust is the right employer for you.
